24/09/2012 201200068 The complainant (Mrs C) raised a number of concerns that her sister, Ms A, reported to her GP practice (the Practice) symptoms of increasing chest, neck and back pain which were not properly investigated.
19/09/2012 201102756  The complainant, Mr C, raised a number of concerns about the care and treatment given to his father (Mr A) during the final days of his life. 
21/03/2012 201100385  The complainant (Mrs C) raised a number of concerns about the investigation and diagnosis of her sister (Mrs A)'s breast cancer by Mrs A's GP practice (the Practice) from May 2010 until November 2010. 
18/01/2012 201002075 The complainant (Mrs C) raised a number of concerns about delays and failures in the care and treatment provided to her mother (Mrs A) by a medical practice (the Practice) between November 2009 and August 2010. Mrs C was also dissatisfied with aspects of the Practice response to her complaints.  
18/05/2011 200904350 The complainant (Mrs C) raised a number of concerns about the care and treatment provided to her husband (Mr C) by Forth Valley NHS Board (the Board) at Stirling Royal Infirmary (the Hospital) from 3 April 2006 until his death on 27 July 2006. Mrs C also raised concerns about the way in which the Board handled her complaint.
24/03/2010 200802819 The complainant (Mrs C) complained about the Dental Practice (The Practice) he was registered with. In February 2009 Mr C complained that they failed to provide agreed treatment and were unprofessional in their behaviour toward him and in they service they provided.
22/04/2009 200602412 The complainant, Mrs C, raised several concerns about the care and treatment provided to her mother (Mrs A) at Stirling Royal Infirmary, following her admission on 29 May 2006.
22/04/2009 200603044 200700888 The complainant (Mrs C) raised a number of concerns about the psychiatric care and treatment of her late husband (Mr C) who suffered from bi-polar affective disorder.
22/04/2009 200701701 The complainant (Mr C) raised a number of concerns about the care and treatment of his 86-year-old father (Mr A).
25/03/2009 200602930 The complainant Mrs C raised a number of concerns about the care and treatment provided to her daughter (Ms A) who had mental health problems.
22/10/2008 200601326 The complainants, Mr C and Ms A, raised a number of concerns that, following a routine laparoscopy investigation for an infertility problem at Stirling Royal Infirmary (the Hospital) on 9 August 2005, Ms A was admitted as an emergency patient to the Hospital on 12 August 2005 and received inadequate care and treatment.

The complainant (Mr C) raised a number of concerns about the treatment he received when he attended the Accident and Emergency Department at Stirling Royal Infirmary (the Hospital) on 24 June 2007, following a road traffic accident.

18/06/2008 200701273 The complainant, Mr C, complained that, despite the fact that Forth Valley NHS Board (the Board) felt unable to treat him, they did not refer him elsewhere. 
21/05/2008 200601583  The complainant (Mrs C) raised a number of concerns about the care and treatment that her husband had received before his death in Bo'ness Hospital on 30 March 2006.
21/05/2008 200602374 The complainant (Miss C) raised a number of concerns about the care and treatment her mother (Mrs A) received in Stirling Royal Infirmary (the Hospital) between her admission on 7 May 2006 and her death on 28 May 2006.
19/03/2008 200601724 The complainant, Mrs C, complained of a lack of local care provision for her son, Mr A, from June 2004 to March 2007.
