The Falkirk Care of the Elderly Outreach Service (FALGO) provides additional support to elderly people in their own homes within the Falkirk District. The Service is hospital based and should be viewed as an extension of hospital care. It has close links with GP Practices, Community Nurses and Social Work Teams.
- The service provides assessment and rehabilitation of people aged 65 and over.
- A multidisciplinary care team of professionals can assess, treat and support you within your own home
- Communication and networking with other agencies
Aims of Service
- Prevent unnecessary hospital admission
- Continue rehabilitation following discharge from hospital
- Promote functional independence
- Improve quality of life

The Service operates Monday to Friday, from 8.30am to 4.30pm

The Intermediate and Rehabilitation Directorate also provides outreach services for the elderly. The NHS Forth Valley - Elderly Outreach Services, Stirling and Clackmannanshire Team provides outreach rehabilitation services for the over 65's in the Stirling and Clackmannan areas, whilst the NHS Forth Valley Area Rehabilitation Service Team provides outreach rehabilitation services for the under 65's in the whole of Forth Valley.
