Stirling and Clacks - Elderly Outreach Services (CARE)

Services

Provides an outreach rehabilitation service for over 65's in the Stirling and Clackmannan areas.  The Service is hospital based but provides rehabilitation for patients in their own homes or in community settings.  It has close links with GP Practices, Community Nurses and Social Work Teams.

  • The service provides assessment and rehabilitation of people aged 65 and over.
  • A multidisciplinary care team of professionals assesses, treats and supports patients in their own homes
  • Communication and networking with other agencies

Aims of Service

  • Prevent unnecessary hospital admission
  • Continue rehabilitation following discharge from hospital
  • Promote functional independence

The service working hours are Monday to Friday, from 8.30am to 4.30pm
