Services

Specialist healthcare services for people with learning disabilities who live in Forth Valley are provided by the Community Learning Disability Team (CLDTs) and the 26-bed in-patient unit at Loch View. The CLDTs are based in a variety of community locations throughout Forth Valley, and Loch View is situated just off Stirling Road in Larbert.

The CLDTs are multi-professionals that provide specialist assessment and intervention at home for people with learning disabilities who have complex health and social care needs including for example, epilepsy, autism, mental health, communication difficulties and/or challenging behaviour. CLDT members also offer support, advice and education to carers to assist them in meeting the health and social care needs of people with learning disabilities. The CLDTs provide links with generic and other specialist health and social care services to ensure equity of access for people with learning disabilities.

Loch View provides care for people with learning disabilities who require specialist or complex health assessment and treatment which cannot, at that time, be met in the community either by generic health services or the CLDTs. Within Loch View, a multi-agency, therapeutic model of care is delivered which promotes rehabilitation and social inclusion. Staff from Loch View work closely with CLDT members.

Integrated Teams - Falkirk, Clackmannanshire and Stirling

The Integrated Teams provide an effective, comprehensive, integrated quality person-centred service to people with a learning disability who have complex care needs. The Teams provide advice and support to relatives, carers and other professional services to enhance support networks for service users, to ensure a quality of life reflective of their own aspirations.

The teams have representation from the following disciplines:- Community Learning Disability Nursing, Occupational Therapy, Physiotherapy, Dietetics, Social Work, Art Therapy, Music Therapy, Speech and Language Therapy, Administration.

The disciplines of Psychiatry, Psychology, G.P. Specialist Liaison for Learning Disability and Additional Support team function as part of the team but are not operationally managed within the team at this time.

Falkirk Learning Disability Team

The Falkirk Learning Disability Team provides an integrated health and social service to people with complex needs living in the Falkirk area.

Referral Process

The team operates an open referral system accepting referrals for service users from the Falkirk area who have a learning disability and complex care needs. The service will work with people who are over the age of 16 years. In the case of transition from childcare services to adult services contact may commence before the age of 16, if appropriate, to ease the process of transition. However transition cases remain the responsibility of childcare services whilst under the age of 16.

The Clackmannanshire Teams do not have a single manager but co-location and close partnership working ensures faster access to and service delivery for clients

Referral Process

An open referral system operates to accommodate referrals from any source by phone, word of mouth, letter to the CLDT or the Community Care Team.  The CLDT accepts referrals for clients with complex needs who are 16 years or above.  Referral forms can be obtained from and returned to:

Stirling Learning Disability Team (SLDT)

The SLDT provides an integrated health and social service to people with complex needs living in the Stirling area.

Local leadership is provided by a single operational Manager, who works in partnership with the Lead Nurse for Learning Disabilities and the Professional Heads of Service for the Allied Health Professionals from NHS Forth Valley. Stirling Council is the lead agency and host the team under its Community Services Care Management structure.

Referral Process

The team will accept referrals for clients with complex needs who are over the age of 16.
