Several months ago the Delayed Discharge Steering group commissioned a review of discharge processes.
As lead officer I set up the Transforming Discharge Processes Group with a wide variety of members from across FV to produce a new Joint Admission, Transfer and Discharge Policy as part of the overall review of service provision in Forth Valley.
A multiagency sub group, chaired by Chris Beech, was established and this group has reviewed practice/ policies from other areas including recent national guidance on Admission, Transfer and Discharge. They have also incorporated the outputs from two stakeholder events held earlier this year.
See the table below for copies of the policy and associated leaflets for your comment so that we can develop the policy further prior to its launch early in 2010. The aim is to implement the new policy over a period of two years to fit in with the development of the new acute hospital at Larbert and associated community hospital developments.
