Clackmannanshire Community Health Partnership

Overview

In Clackmannanshire the CHP Committee has agreed to work on four key priorities during 2007.

1. Improving the health of local communities in Clackmannanshire

The key national and local priorities are to tackle coronary heart disease, poor diet, alcohol abuse, smoking, increase physical activity and to improve mental health.

We know that poverty and deprivation are linked to ill health and our priority is to work with those at most risk of becoming ill.

Anticipatory Care means spotting risk factors and tackling them through health promotion and prevention, rather than waiting for the illness to take hold. This approach is considered by clinicians to be vital to future wellbeing. An Anticipatory Care project is being planned and will be based in Tullibody.

2. Extending joint working

Working in partnership with other people and organisations is seen as very important. Our key priorities are:

3. Delivering more and better services locally in Clackmannanshire

We are committed to improving waiting times and people's access to services, particularly for those who suffer from long term conditions.  Clackmannanshire CHP has agreed that this year we will target:

4. Demonstrating that the CHP is making a difference

This is a way of monitoring how effectively a CHP is performing. It is about ensuring measures are in place to meet any targets which have been set and being able to show that our Partnership is making a difference.
