Primary Care Pharmacy Services
Mental Health In-patient Services
The Clinical Pharmacy Team provides pharmaceutical care as part of the multi-disciplinary team to:
- Adult Mental Health wards (including the Intensive Home Treatment Team)
- Old Age Psychiatry wards
- Learning Disability wards
- Community Mental Health teams
The servies provided include:
- Provision of a specialist mental health medicines information service
- Pharmaceutical care of individual patients
- Medication reviews where required
- Medication education to staff/patients/carers
- Assessment for self-administration of medication
- Co-ordination of a clozapine dispensing service to patients throughout Forth Valley
- Participation in One-Stop dispensing in designated admission wards
- Development of evidence based guidelines
Patients may be referred by consultants and nursing staff for information or advice on their medicines, for a self-administration assessment, or medicine review.

Vaccines
The Clinical Pharmacy Team provides a medicine information service for all vaccine enquiries throughout Forth Valley.
The service provides:
- Advice on the storage and transport of vaccines
- Advice and participates in the planning and organisation of new national immunisation schedules and catch-up programmes.

Opening Hours
Monday - Friday, 8.30 am -5.00 pm, except Christmas Day and New Year's Day.
Outwith normal opening hours, a duty on-call Pharmacist can be contacted via
the Forth Valley Royal Hospital switchboard - (01324) 566 000.
Pharmacy Medicines Information ServicesEnquiries are accepted from health care staff, patients and carers,
and a range of medicine fact sheets is available.
Prescribing Support Team
The Prescribing Support Team consists of
-CHP Lead Pharmacists
-Primary Care Pharmacists (based in GP Practices)
-Prescribing Support Technicians
-Administration Officer (medication review)
-Project manager (community pharmacy smoking cessation and sexual health
service)
The team promote rational and cost effective prescribing and provide information and practical advice relating to prescribing and pharmaceutical issues to GPs, Nurses, community pharmacists & other primary care staff.
Services include:
- Prescribing analysis
- Supporting prescribing initiatives
- Formulary drug choices & guidelines
- Information provision (news letters, enquiries)
- Supporting community pharmacy services (smoking cessation, sexual health services, palliative care network)
- Undertaking medication review (within GP Practices)
