Community Health Partnership

NHS Forth Valley provides a wide range of community based health services which are delivered in homes, health centres and clinics across Forth Valley. These include services delivered by health visitors, district nurses, physiotherapists, podiatrists and speech and language therapists. Services are currently managed within Community Health Partnerships which are run in partnership with local councils who are responsible for the delivery of social work and social care services.

There is currently a joint Community Health Partnership covering the Stirling and Clackmannanshire Council areas and a Community Health Partnership for the Falkirk Council area. NHS Forth Valley has a single management team responsible for overseeing the development and delivery of community health services across Forth Valley. 

Clackmannanshire Strategic Plan

The 3 Year Strategic Plan (link below) sets out the strategic direction for the Clackmannanshire Community Health Partnership for the period 2009-2012.   Its purpose is to be clear about our vision for the CHP.   This vision is to work in partnership to improve the health of our local communities.

Consequently, our Plan is all about working in Partnership:

• Partnership to tackle poor health and address the causes of poverty
• Partnership to deliver better services for older people and for children
• Partnership with clinicians to improve the quality of those services which people receive from GPs, Pharmacists, Dentists, Community Nursing and Community Health Services such as Podiatrists and Physiotherapists.
• Partnership with patients, the public and voluntary sector groups to make sure they are involved in everything the CHP does, and can tells us what they want from their local Health Service.

The purpose of the Plan is also to be clear about the priorities which the CHP considers to be the most important to tackle over this period. It will guide the work of the CHP Management Team and will be the key document against which the CHP will measure its ongoing success.
